U10 CYSA FOUNDERS' CUP APPLICATION

to access the online application you the applicant is required to read and agree to the following important information:

In order for the U10 CYSA Founders' Cup application to be considered for acceptance, all required components of the application must be competed and received in the CYSA State Office no later than 5:00 PM on Monday, November 1, 2010.

In order to expedite their team's acceptance, CYSA Founders' Cup applicants are STRONGLY encouraged to apply by using the online application process. 

THE THREE STEPS REQUIRED TO COMPLETE THE CYSA FOUNDERS' CUP APPLICATION:

STEP 1 - ENTER

·          Complete the online CYSA Founders' Cup entry form. (Applicants requiring a hard copy application should contact the Programs Department staff at the CYSA office for instructions)

·          Enter the official team name by listing the name of the league, team name and 6-digit team number as it is listed on the team's approved CYSA roster. 

·          DO NOT USE A "NICKNAME" FOR THE TEAM. All cup records and files are maintained by the league and the official team name.

·          Applicants can return to the team application to revise and/or complete information by entering the USER ID and PASSWORD they selected when completing the team contact information section of the application.

STEP 2 - PAY

·          To expedite the application and to receive immediate confirmation of payment by email, applicants are encouraged to pay the entry fee online by using VISA, MasterCard or Discover card.  Applicants opting not to pay online may submit a check or money order payable to CYSA.

·          ENTRY FEES OF $400 MUST BE RECEIVED BY 5 PM NOVEMBER 1, 2010.

STEP 3 - SUBMIT ROSTER

·          Submit a copy of the approved, official CYSA roster (goldenrod) to the CYSA office.  To expedite the team's acceptance, submit the roster by scanning and emailing the attachment to kkillion@cysanorth.org or by faxing a copy of the roster to the CYSA office at 925-426-9473. 

·          If paying by check or money order, please include a copy of the roster with the payment - do not email or fax a copy of the team roster. 

·          THE ROSTER MUST BE RECEIVED BY THE 5 PM NOVEMBER 1, 2010 DEADLINE.  LATE SUBMISSIONS DUE TO DELIVERIES RECEIVED AFTER THE DEADLINE OR ILLEGIBLE FAX TRANSMISSIONS WILL NOT BE ACCEPTED.

TEAM OFFICIALS ARE REQUIRED TO READ AND AGREE TO FOLLOW ALL CYSA FOUNDERS' CUP COMPETITION DIRECTIVES, RULES AND REGULATIONS. THERE ARE PENALTIES FOR TEAM WITHDRAWALS AND FORFEITURES WHICH ARE SPECIFIED WITHIN THE RULES DOCUMENT. THESE DOCUMENTS CAN BE LOCATED IN THE CYSA TEAM MANUAL AND ON THE CYSA WEBSITE.
