Note: Players under the age of 18 who play for a CSAN affiliated team must sign this form and also obtain the signature of at least one parent
CYSA Membership Form #1601 (Season 2010/2011)
Note: This form requires a wet-ink signature: This form MUST be signed by the Parent/Legal Guardian OR a Player age 18 or over. Players who turn 18 during the seasonal year must have a new form filled out. Players who are 18 MUST sign their own form. The wet-ink signature must be in a color ink other than black if the form is downloaded from the CYSA website. Two copies of this form must be turned into your League/Club Registrar with the appropriate fees. You may make a copy of the signed form and turned the original and the copy in or you may turn in two signed copies of this form.

Governing Policy Provisions
US Soccer Policy 601-6 - International Clearances provides in full:
For a player who comes to the United States on or after the player is 12 years of age, the player must request and receive an international clearance from the Federation. However, an international clearance is not required of a player who is 12, 13, 14, 15, or 16 years of age if the player and the player's parent or guardian signs a statement that the player has not signed a professional contract or received money from any team for playing soccer.
United States Youth Soccer Policy on Players and Playing Rules (December 1, 2006), Rule 207 provides in full:
Rule 207. INTERNATIONAL CLEARANCES
(a) For a player who is at least 17 years of age and comes to the United States on or after the player is 12 years of age, the player must request an international clearance from the Federation.
(b) For a player who is 12, 13, 14, 15, or 16 years of age and comes to the United States on or after the player is 12 years of age, an international clearance is not required. However, the State Association through which the player is registered may require that -
(1) the player's parent, parents, or legal guardian complete and sign a statement that the player has not signed a professional contract or received money from a professional team for playing soccer; and
(2) the statement be (A) retained by the club or league with which the player is registered, or (B) filed with the State Association.
CYSA International Clearance Compliance Policy
(a) International clearances are required only for Division 1 players who are 17 years of age or older on or before the final State Cup game for that player's age group and gender who came to the United States after the player was 12 years of age and who are participating in State Cup competition.
(1) The international clearance must be obtained from the Federation on or before the date the final State Cup game is played for the affected player's age group and gender or the player will NOT be eligible to continue in state, regional, or national cup play;
(2) It is the responsibility of the player, the team, and the league to insure that that any required international clearances are appropriately and timely obtained;
(3) When required the International Clearance Form must be sent to the CYSA office for the authorization of the State Association. The CYSA office will forward the original to US Soccer and retain copies in a file.
(b) Each league may require for Division 1 players who are 12, 13, 14, 15, or 16 years of age and who came to the United States after the player was 12 years of age that the player's parent, parents, or legal guardian sign a statement that the player has not signed a professional contract or received money from a professional team for playing soccer.
(1) The statements are retained by the league or, if the district requires, the district.
International Clearance Inquiry Form (Optional)
1. Are you over the age of 17 years?
2. Did you come to the United States after you were 12 years old?
If the answer to both questions 1 and 2 are "YES," then an international clarance is required. In that case, the International Clearance Form must be sent to the CYSA State Officce for the authorization of the State Association. The CYSA State Office will forward the original to US SOccer and retain copies on file.

Youth to Provisional Senior Information
This is the time of year when many new youth teams are forming. Because it is more prevalent than ever for youth players to also participate on adult (senior) teams, we are asking for your cooperation in our efforts to safeguard our players' youth eligibility.
It is essential that we inform the players, as well as the coaches of these youth players, of the potential negative consequences that can befall them and their youth team if the proper protocol is not followed. Failure to follow the procedure can lead to disqualification of the player and forfeiture of any games in which their youth team participated.
Please refer to CYSA Bylaws sections 3:05, 3:06 and CYSA PIM 78-4 and PIM 90-1 for clarification of these rules regarding youth players participating with or against senior (adult) players.
In order for a youth player to participate and/or register with an adult soccer organization the Youth to Provisional Senior form (form 1612 revised 12/2006) must be completed. This requirement applies to any player that qualifies to register as a youth player. In accordance with current policies, if the youth player is not currently registered or rostered with a youth team, the youth player must still obtain approval from their District Commissioner/Registrar in order to protect any future consideration for playing at the youth level.
Form 1612 (rev. 12/2006) may be obtained from the youth league registrar or on the CYSA web site. Any prior revisions of the form are now obsolete. It is the player's responsibility to obtain all of the necessary signatures prior to submitting it to the CYSA State Office.
It is the responsibility of the league to ensure that their players and coaches are aware of this policy. Please make sure you share this information with each of your coaches who may have players who plan to register or participate as a senior player.
