Acknowledgement of CYSA Roster Exception for CYSA State Cup/US Youth Soccer National Championship Series Play

CSAN Player & Parent Waiver Form Note: Players under the age of 18 who play for a CSAN affiliated team must sign this form and also obtain the signature of at least one parent

CYSA Membership Form #1601 (Season 2009/2010) Note: This form requires a wet-ink signature: This form MUST be signed by the Parent/Legal Guardian OR a Player age 18 or over.  Players who turn 18 during the seasonal year must have a new form filled out. Players who are 18 MUST sign their own form.  The wet-ink signature must be in a color.  Two copies of this form must be turned into your League/Club Registrar with the appropriate fees.  You may make a copy of the signed form and turned the original and the copy in or you may turn in two signed copies of this form. Please DO NOT mail or send this form into the CYSA State Office.
