Note: Players under the age of 18 who play for a CSAN affiliated team must sign this form and also obtain the signature of at least one parent

Note: This form requires a wet-ink signature: This form MUST be signed by the Parent/Legal Guardian OR a Player age 18 or over.  Players who turn 18 during the seasonal year must have a new form filled out. Players who are 18 MUST sign their own form.  The wet-ink signature must be in a color ink other than black if the form is downloaded from the CYSA website.  Two copies of this form must be turned into your League/Club Registrar with the appropriate fees.  You may make a copy of the signed form and turned the original and the copy in or you may turn in two signed copies of this form.

Please DO NOT mail or send this form into the CYSA State Office.

Note: A request for Interstate Permission Guest Play requires the form to be completed and submitted into the CYSA State Office 14 business days prior to travel as a Interstate Guest Player.  If this form isn't completed in the 14 Business day periord for Interstate Guest Play, the permission to travel as a Guest Player may not be granted.
