WORD League Affiliation Agreement 2010/2011 Seasonal Year 

Note: 2010/2011 LEAGUE AFFILIATION AGREEMENT (Form 8201) MUST be signed by two (2) League Officials and submitted with a League check for $200.00 payable to CYSA.

  1. League Affiliation Agreement form is signed by two League Officials.

  2. Check for $200.00 payable to CYSA.

  3. Copy of the most current League operating procedures. (Constitution, Bylaws, General Rules)

  4. Must submit proof of current good standing with the IRS. (Internal Revenue Service). In regards to your Leagues 501(c) (3) status is current and in good standing with the IRS. Contact information, IRS. Phone# 1-800-829-3676, (877) 829-5500. Website http://www.irs.gov/

  5. Must submit proof of current good standing with the Secretary of State. In regards to article of incorporation is current and in good standing with the Secretary of State. Contact information, Secretary of State, phone # (916) 657-5448, Website http://www.ss.ca.gov/

  6. Copy of Leagues current Directors and Officers Insurance.

The above documents should be sent to you District Commissioner no later than February 15, 2010 (or earlier date if set by your District Commissioner), and must be submitted by the District Commissioner to the CYSA State Office no later than March 1, 2010.
