NOTE:  This is a Fill-In & Print form and obtain the required wet-ink signature:  This form MUST be signed by the Parent/Legal Guardian OR a Player age 18 or over.  Players who are age 18 or over MUST sign their own form.  The wet ink signatures should be in a color.  Two copies of this form must be turned in to your League/Club Registrar with the appropriate fees.  You may make a copy of the signed form and turn the original and the copy in or you may turn in two signed copies.  Contact your League/Club Registrar if you have any questions.

Note:  This form replaces forms 1608 Player Add Form, 1609 Player Release Form, 1610 Player Transfer Form & 1626 Adult Add Form.

Note: A request for Interstate Permission for Transfer/Release/Guest Play requires the form to be completed and submitted into the CYSA State Office 14 Business days prior to travel as a Guest Player. Forms that are not submitted in the 14 Business day period for Guest Play will not be guaranteed permission to travel as a Guest Player.
