In order to complete the
Team Official Registration & Risk Management Disclosure form, you
will need to provide a form of identification. Identification can be
in the form of:

Driver License or
Identification Card
-
Passport
-
Social Security Number
(optional)
-
Military I.D.
-
School I.D.
-
You
MUST need to answer all
six (6) questions in the form's "Important Registration Questions"
section.

If you answer "YES" to any
of the one or all six (6) questions, you will need to complete the
back (reverse) part of the form. To type into the back part of the form

All forms with one ore more "YES"
boxes checked
need to be submitted to the CYSA State Office within 24 hours:

that is marked CONFIDENTIAL

All forms
with NO "YES"
boxes checked need to have two copies printed and signed and
submitted to your League/Club.

Mail your two (2) copies of
the form to your League/Club of registration if you have answered
"NO" to all six (6) questions.

DO NOT send your "NO" marked
Team Official Registration and Risk Management forms to the CYSA
State Office.

It is your responsibility by
submitted your Team Official Registration and Risk Management form
to the proper League/Club.
